The Division of Injury Prevention seeks to reduce the number of serious injuries and injury-cause deaths in Jersey City. Focus areas include violence prevention, community mental health and suicide prevention, overdose prevention, and road safety. The Division of Injury Prevention manages and participates in a number of programs and initiatives, including Violence Prevention, community mental health wellness and training, the Vision Zero task Force, and mobile health initiative. 

The Health Outreach Project provides outreach, including showers, toilets, and other health services to individuals according to project guidelines. The program, currently serves the general public with specific focus on persons experiencing homelessness. 

The Division of Injury Prevention is looking for a Health Outreach Aide who will report to the Health Outreach Specialist assigned to this project. The Health Outreach Aide will support the operations of the health outreach program.

  • Driving the Health Bus and other vehicles to enable outreach throughout Jersey City. 
  • Operate the mobile outreach program at designated locations throughout Jersey City. Daily operations include welcoming and managing services to program participants, cleaning and maintaining equipment, including the bathroom units, and generally ensuring that the program provides its services to the public. 
  • Supports service of equipment and receives deliveries of supplies. 
  • Completes daily program reports, maintains program files, and performs data entry. 
  • Tracking inventory and restocking program supplies. 
  • Engages program participants in conversation, especially about health and wellness issues. 
  • Supportive counseling of program participants. 
  • Administers surveys and performs individual needs assessments. 
  • Makes service referrals and educates program participants about services. 
  • Promptly reports services, issues, and problems. 
  • Phone, email, and in-person communication with other staff. 
  • Submits invoices, travel reimbursements, and expense reports.
  • Under direction, assists with other programmatic activities of the Division. 
  • Under direction, makes contact with partner organizations supporting the Division’s work. 
  • Participates in training, workshops, webinars, and other assigned professional development. 
  • Assists with other duties and projects as assigned. 
  • Is aware of and complies with the Policies and Procedures of the City of Jersey City, Dept. of Health & Human Services, and the Division of Injury Prevention.

Required Educational Background: High school Diploma. Associate or Bachelor’s Degree is a plus, especially in or with coursework in the fields of human services, social services, or psychology. 

Required Experience: Minimum one year of administrative and/or programmatic experience. Must possess a valid New Jersey Driver’s License. Must have or acquire within 90 days of employment a Commercial Driver License Class B or higher. Must be proficient with a computer or mobile device for data entry.

  • Ability to carry out tasks quickly, competently, and within the designated time frame.
  • Competent in computer applications such as Microsoft Office, including Outlook, Word, and Excel.

Preferred Education/Skills/Qualifications: 

  • Fluency (speaking, reading, and writing) in English is required. Ability in Spanish, Arabic, or other languages commonly spoken in Jersey City is a plus. 
  • Demonstrated interest and/or knowledge of community/public health, mental health, or any of the Division’s subject areas
