Service Coordinator NJ

Elmy's Special services is looking for Service Coordinator in NJ. As a Service Coordinator, you will monitor and coordinate Early Intervention services, working to ensure family satisfaction with service provision. Responsibilities include, but are not limited to:

• Act as a liaison between families and providers
• Develop plans and strategies to meet the family needs
• Advocate for families to gain access to services
• Assist family with community resources
• Guide family in transitioning to other programs
• Timely documentation of Service Coordination activities via electronic database.

Qualifications:

• Bachelor’s degree in Health or Human Services or
• Associates degree in Health or Human Services with two years - experience working directly with children from birth to three years of age
• Bilingual a plus
• Must have a minimum of 1 year experience as an Early Intervention Service Coordinator

Job Type: Full-time

Pay: $36,000.00 - $41,000.00 per year

Schedule:

• 8 hour shift
• Monday to Friday

Experience:

• Support Coordinator: 1 year (Preferred)
• Individualized Support Plan: 1 year (Preferred)
• in-home service: 1 year (Preferred)
• Microsoft Office: 1 year (Preferred)

License/Certification:

• COVID Vaccine (Required)
• Driver's License (Preferred)
